Submit Visit Feedback

Sign in to the patient portal, rate a visit, and choose whether the feedback may contribute to the clinic's public website.

Submit visit feedback when your clinic sends a portal feedback link after a visit. You will sign in with the clinic's email or SMS verification code before the survey opens.

Who does this: Patients and caregivers.

Steps

Open the feedback link from your clinic. It takes you to the clinic's patient portal.

If asked, enter your email address or phone number and the one-time verification code. After sign-in, the portal returns you to the feedback survey.

Select a star rating.

After you select a rating, the feedback details step opens. Answer each required follow-up question that appears.

Use Additional note to clinic to add an optional note. Select the consent checkbox only if you want the rating to count toward the clinic's public rating and response total. The clinic may also publish your note and abbreviated display name after review. Leave the checkbox unchecked to keep the entire response private.

Select Send feedback.

Select Done on the thank-you screen, or continue to the public review option if one appears.

Common pitfall: Feedback links may expire after a set period.

Troubleshooting

The link is expired. Ask the clinic if they can send a new feedback link.

The feedback page asks me to sign in. Complete the portal verification step with the email address or phone number your clinic has on file.

I cannot submit. Select a rating and answer any required follow-up questions.
