Sign in to the patient portal with a code.
Sign in to the patient portal to view appointments, paperwork, messages, and profile details.
Who does this: Patients and caregivers.
Steps
Open your clinic's patient portal link.
If your clinic offers both, choose Email or SMS.
Enter your email address or phone number, matching the method you chose.
Select Send verification code.
Open the code from your email or text message.
Enter the code on the portal page.
Your portal home page opens.
Common pitfall: Use the email address or phone number your clinic has on file.
Troubleshooting
I did not get a code. Check your email and phone, then request another code.
The code does not work. Request a new code and enter the latest one.
My child's records are gone from my portal. Parent and guardian access ends on the child's 18th birthday. From then on the records belong to their own portal account, and they sign in with their own email address. If the clinic does not have an email for them yet, contact the clinic and ask them to add one.
