Create Consent Templates
Create a reusable consent template for patient signatures.
Create consent templates when your practice needs reusable documents for patient signatures.
Who does this: Clinic administrators.
Steps
Open Settings.
Open Templates.
Select Consent Templates.
Select New consent template.
Enter the template name on New Consent Template.
Choose the creation option shown on the page, such as Create New.
Choose how to create the consent:
- For Create New, write the consent directly on Letter-sized pages. Add, duplicate, or remove pages, and place patient or front-desk signing fields on the same page while you write. When you paste long text or import a Markdown, HTML, or Word file, the designer automatically continues the content onto additional pages. Editor exports include every page and use the template name for the downloaded filename.
- For Upload PDF, upload the finished PDF and place signing fields over its locked pages. Use the page thumbnails, zoom controls, drag handles, and arrow keys when a field needs precise alignment.
Select Preview to review the patient-facing document.
Select Save Template.
The consent template is available for intake packets.
Common pitfall: Preview the consent before adding it to a packet.
Troubleshooting
The template will not save. Confirm the template has a name and at least one signing field. For a template created from scratch, split an unusually large table, media item, or formatted block if the designer reports that it cannot fit on one page.
The preview looks wrong. Return to the page designer, adjust the text or field position, and preview again. Field coordinates are saved relative to the page, so zooming does not change their final position.
