Edit Patient Details

Steps
Open Patients from the main menu.
Find the patient and open the actions menu.
Select Edit Patient.
Update First Name, Last Name, or Date of Birth in Edit Patient if needed.
Open sections such as Guardian Contact, Emergency Contact, Insurance, or Provider & Source.
Make the needed changes.
Select Save Changes.
The patient record updates.
Common pitfall: Minor patients may require guardian contact details before saving.
When a minor turns 18
A parent or guardian portal link stops working on the patient's 18th birthday. From that day the chart moves to the patient's own portal account, so the patient needs their own email address on file. It has to belong to them, not the guardian, because it becomes how they sign in.
Zcribe watches for this and sends reminders while the patient still has no email of their own:

The guardian stops receiving messages on the birthday, because their link to the chart has ended by then.
Clinic reminders arrive by email at your organization's address and link straight to the patient's chart. Guardians may also get a text, which never contains the patient's name, only a link to sign in.
Act on the birthday reminder. It is sent once, not repeated. If nobody adds the patient's email, that chart cannot be opened from the portal by anyone: the guardian's link has ended and the patient has no way to sign in.
To fix it, open the patient and add their personal email under Edit Patient. If the guardian should keep access instead, grant it from the Portal access section of the patient chart. Either one stops the reminders.
Troubleshooting
The form will not save. Check required fields and valid email addresses.
I cannot edit the patient. Ask an administrator to confirm your access.
